Each admittance card will be personally assigned and is non-transferable. Please be aware that it will be necessary to present the card at each service, with a photo I.D.

BOOK OF MEMORY
Temple Beth Sholom's Book of Memory is distributed at the Yizkor Memorial Service on Yom Kippur afternoon. It is a fitting tradition to create a living memorial to honor your departed loved ones by contributing to the Temple on this occasion. The amount you give, be it large or small, is entirely at your discretion.
If you wish to have names included in the book, please return the Book of Memory form, along with your contribution, to the Temple by August 8, 2012. For more information, contact Lois Kotzen in the Temple office - 305.538.7231 ext. 253. You must return the form to have names included.
